  • A BevMo! Store receipt with a survey invitation.
  • A basic understanding of English.
  • Be 21 years or age or older and a legal resident of the United States to enter the sweepstakes.

Instructions:



  1. Visit the customer survey site at BevMosurvey.com.
  2. Click the forward arrow to continue.
  3. Enter the following information from your store receipt: date of visit, 15-digit access code, and the 11-digit ClubBev! ID number.
  4. Click on the forward arrow to proceed.
  5. Select the time of day you visited the store.
  6. Think about your shopping experience and answer a few questions.
  7. Answer all questions truthfully.
  8. Fill in your contact information for contest purposes.
  9. Submit the survey.

  1. I live in Vacaville, calif. and the bevmo in my town was out of my trader vics hot buttered rum, so I tried the one in sac. they too were out but informed me the bevmo in Gilroy had some. I got the number, called and the manager mary jane was so helpful and polite but we couldn’t get it to order correctly on the web site so another lady from their called me, her name was laurel and she also was so helpful and extremely friendly and polite we got my 8 containers of buttered rum ordered that day and I received them a day latter. what a bad situation from the Vacaville store was, turned out to be a wonderful end result from the Gilroy store. this is the second winter bevmos had gotten me thru,
